They can be pulled from you whenever an MTI (Military Training Instructor) requests them. An MTI is the Air Force's equivalent of a "Drill Instructor." A 341 would be pulled from you if you were found to be in non-compliance of Air Force rules, regulations, or code--uniform violation--your boots were not shined to their liking!!! You could also have a 341 pulled if you exceeded their expectations, highly unlikely! It's been so long but I think they are called commendation/discrepancy forms.
