So far this year, CP has had two derailments, one in Elgin, IL, and another on a branch line a few days ago, which shut down the branch. CSX also had a recent derailment, which also led to a shutdown of a route. Northfolk Southern is cleaning up after a wreck on it's northern Ohio Line (EX Nickel Plate) and an engineer is being hospitalized after a wreck on the New England Central. This is the most wrecks I've ever heard of in such a short amount of time. this month seems like the worst when it comes to wrecks and derailments.
The CN had 19 difco dump cars derail on this ramp, and two actually fell off, just missing the street below (note, not this street, one farther back):

The Missabe Road: Safety First
CSX put 7 or 8 cars on the ground in Canton, NY (Q620) yesterday. No word as to the cause.
