I noticed that high level platforms might be designed in a way that railroad trespassers cant climb, or jump on to them. Is that true? If so how do they expect people who fall on to the tracks by accident to get back up if there is no side step or ladder.

I don't know how it's done elsewhere, but on the CTA, there are ladders for employees at the ends of the platforms that go to track level. Of course, avoiding the third rail would also be a major consideration.
On the IC (Metra Electric), the main line was on an embankment and the stations had a short stretch of walkway at track level between the platform and the stairs down to the exit at ground level.
