Does any know what the US safety record is relative to brake failure?
To my knowledge there is no specific statistic in railroad metrix that identify brake failure. The best you can do is to peruse the NTSB and FRA train accident reports which identify causes of accidents that result in the death of employees, passengers or the non-railroad public.
