You should add Baltimore to your list - you can hit the B&O Museum and take in a game at Camden Yards...
West of the river I think I'd like to hit the Nevada Northern and the California State museum.
East of the river, I've hit a lot of the high points already, although the Illinois museum piques my interest.

Many places to visit east of the old man river. I think I'd go see the General in Kenesaw, GA (even though there is no active steam train there).
I have not been to any of these places for many (tooooo many) years, but:
The Tennesee Valley Railway Museum in Chattanooga, TN.
New Hope and Ivyland in PA.
Spencer, NC.
Scranton, PA.
Ft Wayne, IN.
Owosso, MI.
Union, IL
There are probably a dozen more that I have not been able to visit yet.

Others have responded just as I would, but I've got to agree.
East of the Mississippi, Strasburg PA, hands down. Everything, and I mean EVERYTHING on a railfan or railmodeler's wish list. I could stay there the rest of my life and never be bored.
West of the Mississippi? Hands-down, the Durango and Silverton, fantastic ride and museum.
I've been to both, east and west, and they're both winners.

Which side of the Mississippi are we talking about?
For east I would pick the Strasburg Railroad and Railroad Museum of Pennsylvania, which are at the same location.
For west I would pick either the Durango & Silverton or Cumbres & Toltec, both narrow gauge lines are amazing examples of surviving steam railroading, and they are coal-fired to boot! (though this has recently got the D&S into a spot of trouble)
