Having a hard time seeing it, maybe it's my Scottpak in the way, or my structural protective gear and helmet??

Alot of stoned or dead passengers if it was pull on an excursion open top train through a tunnel. (cough......) Is that blood?

I would think that it might not be very conservative on fuel.

And I agree on the air packs and protective gear,since that
is rather highly volatile fuel.

Oh, I forgot, you're a PSD man.

You're looking at a built Cummins, probably with twins. (These can make even the best Diesel Dynamics PSDs look a bit slow...) Looks like the infamous "Black Dodge" (hint: look at www.smokinya.com -- subtle URL, no?)

Many of these trucks have 'smoke switches' which intentionally intensify the unburned-smoke thang. Foxworthy et al. may have commented on this (and, if not, probably should!) It's theater.

You see smoke like this at tractor pulls, too -- and I might add at lawnmower pulls (does 750hp out of a 3-cylinder Kubota start to interest anyone?)

Yes indeed, you COULD put it on rails, with some care, and yes, as jruppert indicates, it would then be a 'high rail to hell'. Wouldn't have too good a factor of adhesion, though...
