Wait, please, before assessing blame. The cause is not yet reported. It could be mechanical, track, both, or other. Four derailments in 10 years means nothing in and of itself. Even track and equipment maintained to the highest standard will experience failure. After a cause is determined, and a pattern established, THEN it is fair to comment. Until then, comments on cause and blame will only encourage responsible users to leave this forum. When they leave, answers to questions will disappear with them.
