Have you programmed the addresses in? or are you using the default value (ie. Adresses 1,2,3 and 4) It sounds like one of the addresses got messed up.

You might want to repeat the process of assigning addresses and see if it starts working.
The physical equipment sounds like its throwing the switch but when addressing if from loconet (your throttle) its not wanting to throw the turnout. This is why I think the address of the forth channel is messed up.
The only annoying thing about the DS64 is that you have to reprogram all four addresses on the unit at the same time.

Do you happen to use JMRI and have loconet connected to a computer? there are some nice diagnostic tools such as the loconet monitor. One thing the DS-64 does is that it sends a loconet message each time the switch is cycled. The message will indicate the turnout address and its status (Closed or thrown). This is one of the reasons i like the DS64's is becuase even the local input buttons will trigger the DS-64 to send the loconet message and throw the turnout. It provides feedback to the system (good for signalling systems).
You may be able to find out via the Loconet monitor in JMRI which address the non-responsive channel is on or if its even broadcasting.
