how does PowerCab measure current draw?

It is a feature built into the PowerCab itself, and that device's firmware.

I am not aware of any mobile decoders that measure current themselves. 

And even if they did, they would need a feedback mechanism such as RailCom or Digitrax's Transponding to feed that information back to the user.

is it thru the decoder or is it total power thru the PowerCab?

if thru the decoder, does it mean all decoders measure current?
