Yes, I think the smoke unit is the culprit. I guess it broke the first decoder.
It is disconnected and at the desk now.
I will email BLI and see if they have a replacement unit.

I wonder if the fan ran all the time anyway? Try unplugging the single wire in the 9 pin plug, which should also be on the same wire as the switch under the cab, so that is rather odd.
The only thing I can think of for the headlight is that somehow the polarity got reversed? A little poking with a meter should confirm that.
This diagram is from a NYC Hudson but may share some of the same assignments as the 8 pin.
BLI still might use #3 & #4 for the headlight? Or as a last resort, email or call them as Rich suggested.

I found the "Loco Guy" thread and maybe there is something you can pick up from there. In his case a BAD SMOKE UNIT was the culprit after he returned the engine to BLI. Go to last responses.

Front Truck = Black Wire = Right Rail
Rear Truck = Gray Wire = Left Rail

I'll open the T1 and get the routings for the wiring for you later this evening. I recall helping Old Loco Guy or such with his BLI a few months ago and that's when I took this photo.

I do not have the Y6b but I imagine the guts are all pretty much the same. PCI was making decoders for BLI. I have a bunch lying around. Hi all!
I had a blown decoder in a Paragon 2 equipped Y6b which BLI sent a replacement for. They wanted the faulty decoder back, so I photographed the decoder in the tender before removing it.
The problem is that the pics are gone.....
Anyone know where the connectors should be? And also which side the insulated wheels should be on the tender....
Hope someone can help.
