I thought that most DCC systems out there today had a function that allowed you to program in a long address without having to set CVs 17 and 18 independently. Out of curiosity, which system do you have?
Anyway, concerning a table, there is a calculator that will give you the proper decimal values to put in CVs 17 and 18 without having to use a table. You may find that calculator, courtesy of a link from the Tonys Trains website, here: http://ruppweb.dyndns.org/xray/comp/decoder.htm

You should go to the QSI internet site and download the manual(s) appropriate for your locos. All the information is included there. The manuals are quite long but are all inclusive.
