HO scale. SD40-2. The decoder(s) are TCS DP2X changed to a Lenz JST-MP just to make sure it wasn't the decoder. Jellybean
David is exactly right (Ask me how I know!) Here's a link to a pictorial of the hardwiring procedure. You don't have to cut the slots mentioned, so don't be intimidated by that. Just use flexible wire and route it well and you won't have any problems.

I just took the cover off of a different model and I noticed quite a difference between the two. One has wires connecting the front and rear and the one does not. Instead it looks like tabs that are soldered. Hope this helps.
I have a brand new Kato SD40 with a decoder installed and when I tried it out it was very jerky and then it just stalls out. Its NOT dirty track or wheels. When I turn on the throttle and hold up one set of wheels off the track it runs and when I switch to the other set... nothing. I know it isn't getting power but I don't know what to do to fix it. It is all wheel pick up. Jellybean
