There were a LOT of switches of this type used in the surface workings of coal mines in Fukuoka-Ken, Japan in the late '50's. (Probably underground, too - but I didn't go into the workings to check .)
Of course, they were only used by four wheeled mine carts, and by tiny industrial locos that didn't weigh much more than a loaded cart. Track gauges ranged from 450mm to 600mm.

I found this while looking for something totally unrelated....thought it was really interesting
The switch is located in Snaefell on the Isle of Man. It would be cool to model this.
