The smoke unit belongs to a K-line K-4 made in 05 or 04

Hello Alex!   Postwar & most Lionel Smoke units have a Piston inside a round chamber that travels up & down & this is what causes the Smoke to come out of the Stack & the piston is usually connected to a lever that is pushed by a Connecting Rod while the Drive Wheel's turn. Now, on Modern Steam & some Diesel Engines a small fan is used inside of the Engine to accomplish the same amount of work that the Piston above used to do. On some American Flyer Engines the Smoke Generator was actually inside of the Tender & a flexible tube was used in between the engine & tender to get the Smoke to the Smoke Stack. This method is not used anymore by the manufacturer's at all. Hope this helps. Take Care.
