Or as Lucius Beebe once wrote aboout a boiler explosion "No water over the crownsheet will do it everytime".

The root cause of the Three Mile Island failure was the steam generator (boiler) running dry due problems with the feedwater pump (injector), which then led to the reactor running dry due to a leaking power operated relief valve (somewhat similar to a safety valve) compounded by problems the reactor's water level sensor (sight glass).

The five-car train's longest nonstop run was less than 90 miles; water could be had at each of its stops.
