I didnt know it was this low when i bought it second hand no tank or regulator.Can i boost the psi up?

A regulator can only limit the pressure coming out. It cannot make more pressure.

I don't know what that means.
I had a Badger compressor and there was no way to adjust the pressure upwards. I don't think 20 psi is adequate for airbrushing.

Exactly, adding a regulator can only decrease air pressure or keep it the same.
Most compressors which cycle have cut in and cut out points adjusted by a spring-loaded screw.
Yours sounds like its continous run as it does not have a tank. Does it have a gauge and if so what is the highest value on it?

Schuco makes medical compressors for a device called a Nebulizer which aids in breathing. My mother uses one, not sure of the brand.
Some have a 50 PSI rating. Others are lower.
