Here NTSB seems to hope that safety management systems might help. From my own experience I have my doubts. To make it work Amtrak needs to develop an error management culture, where everyone has to report and document his own mistakes and those of others perhaps anonymously. How that is going to work in an atmosphere were the slightest rule violation is followed by harsh disciplinary action I don't see. Regards, Volker
