You never know when it will happen.  Two guys just doing some track welding and then an explosion.

Very sad news. 

Acetylene and Oxygen can be extremely dangerous if they leak, especially in combination.

Oxy-Acetylene is the most dangerous form of metal working tools.  A 'back fire' can work its way back to either supply tank and cause an explosion.
