From the arial photo, looks like the operator went into the curve at a far higher speed than allowed. Note that this happened on completely private-right-of-way, and this system does include some streetcar operation on lanes shared with general traffic.
A newsfeed I read earlier today said that the tram went through a 12mph-limied switch at some four times that speed. (I don't recall the site of the feed, and a Google news search did not turn up the specifc URL)
