<p><span style="font-family: verdana,geneva;"><span style="font-size: small;">The latest news is that the engineer admitted that he knew he was going too fast. Excessive speed could lead to climbing the outside rail on a curve. Such climbing could also be aided by a gage defect such as Paul mentioned above and / or a kink at a rail joint. I have not heard any explanation of why the engineer was going too fast. </span></span></p>
