Did you buy it new, or from someone who might have tinkered with other CVs?
I would try doing a full reset of the decoder, and then resetting the locomotive address. There may be a bogus value in there somewhere. I have one of these myself, and it is nicely behaved.
It takes an iron man to play with a toy iron horse.
Or you could shoot a video of it and then play it in slow motion.
How long have you had the loco and how often has it been run?

I have four of these- two QSI from the factory (one reed and 1 pot), 1 non-sound from the factory with the sound board from an 0-8-0, and one with a LokSound.
All of them run great at slow speeds. Best slow speed steam I have. Favorite yard engines.

My loco is brand new form Walthers, purchased in August 2006. Has the upgrade chip from QSI. I can reset it, but the CV 2 will go to 32, so nothing will change in the slow motion. The loco has 1 hour total running, always with the same failure. Will be useful to me that "1948PRR" will send me a list about the main CV´s to fix. Thanks
When you set CV 2 to a lower value, did you then read back the new value successfully? I'm wondering if you've "locked" the decoder, and you're not really setting the CV when you program it.
If that's not the problem, try the reset, and verify that the address has been reset to 3. If the engine still will not run slowly, you may have a faulty decoder.
Yes I can read the new value. Tonight I´ll reset the loco, but I feel difficult to fine tune up the CV in a QSI, to get just an smooth start and steady motion. I listen advices.Thanks
Yesternight I reseted the engine, and lubricated axles and side roads. The model improved 30%, so I´ll let it in that condition. Beside, will be useful to me that "1948PRR" will send me a list about the main CV´s to fix in a QSI decoder. Thanks all.

My Bro in law just bought a 1966 Sattelite. Same color as my 1966 Coronet. Way cool!
Also have my layout torn up to install a helix and future second level.
