Having some problems with my Zephyr

I thought today was gonna be good because I wired a section of track and was ready to use my Zephyr, now I don't know what is going wrong.

First - I tested the track by placing a quarter over the rails and most times it flashed the walking Os but then all of a sudden it would go off would repeating green pulses. What does that mean?

Secondly - when I went to try to program one of my engines I got as far as inputing a 4 digit number after I pressed the CV-WR button all I got was a  d  nd message

 

Third - when I tried to run a loco without a decoder I went through the steps as soon as the loco was placed on the tracks it had a "mind of its own" it went really fast down the track and could not be controled/stoped by the throttle.

 

I thought this was going to be an easy part

Any help would be great
