Here is a link to some old outdated (I.E.-April 2000) Union Pacific Conductor Forms I scanned after one of my mom's college friends husband sent me. Feel free to use them on your home layout for a more prototypical feel.

The First one is a TRACK and TIME / TRACK PERMIT / FOUL TIME form. The next is a TRACK WARRANT, followed by a CONDUCTOR REPORT form. The last one is from a FREIGHT CONDUCTOR'S TRAIN BOOK.
All booklets are from a Union Pacific Conductor [will remain anonymous for website purposes], courtesy of the Union Pacific Railroad. If you aren't a UP fan, I'm sure some photoshopping could remove the ugly letter combination at the top of the CONDUCTOR REPORT.
